More than 250 million people across the Asia-Pacific region live with a rare disease, and for the overwhelming majority of them, the defining feature of that experience is not the disease itself but the wait: years, often a decade or more, between the first symptoms and an accurate diagnosis, followed by a separate and frequently longer wait for access to any treatment that exists. This diagnostic and treatment gap is not a peripheral problem within regional healthcare policy; given the sheer number of patients affected, it is one of the largest unaddressed public health challenges in the region, and it persists despite genuine advances in genomic technology that have made rare disease diagnosis, in principle, faster and cheaper than at any point in medical history.
The gap between what genomic diagnostic technology can now do and what patients across Asia-Pacific actually experience is substantial and worth stating plainly. Whole genome and exome sequencing, capable of identifying the genetic basis of thousands of rare conditions, has become dramatically cheaper and faster over the past decade, to the point where sequencing cost is rarely the binding constraint on diagnosis in wealthier health systems. Across much of Asia-Pacific, however, the binding constraints are different: insufficient genetic counseling capacity to interpret results and guide families through decisions, a shortage of specialists trained to recognize rare disease presentations in the first place, and fragmented or absent referral pathways connecting primary care physicians, who see the first symptoms, to the specialized centers capable of ordering and interpreting genomic testing.
Even where diagnosis succeeds, treatment access frequently does not follow. Orphan drugs, developed specifically for rare conditions, are priced to recover development costs across small patient populations, which typically means costs that are prohibitive without either national reimbursement schemes or substantial patient assistance programs. Many Asia-Pacific countries lack formal orphan drug pathways within their regulatory and reimbursement systems entirely, meaning a treatment approved and reimbursed in the United States, Europe, or Japan may simply have no formal route to approval, let alone affordability, in Vietnam, Indonesia, or the Philippines. Patients and families in these markets are frequently left navigating compassionate-use programs, medical tourism, or informal importation, none of which constitute a sustainable system.
Government-led initiatives across the region show real but uneven progress. Several countries, including Taiwan, South Korea, and increasingly China, have established dedicated rare disease registries and orphan drug approval pathways that meaningfully shorten the gap between international approval and domestic access. India’s rare disease policy framework has expanded in recent years, including a national fund intended to support treatment costs for patients with certain rare conditions, though implementation has been criticized as underfunded relative to the scale of need and geographically uneven in practice, concentrated around a small number of designated centers of excellence that remain out of practical reach for patients in smaller cities and rural areas. Southeast Asian nations, with less-developed genomic diagnostic infrastructure and generally smaller healthcare budgets relative to population, lag furthest behind on both diagnosis and treatment access.
The biopharmaceutical industry’s role in closing this gap is more complicated than either pure critique or pure partnership narratives suggest. Several multinational orphan drug developers have launched access programs specifically targeting Asia-Pacific markets, ranging from tiered pricing structures to donated-treatment programs for diagnosed patients who cannot afford standard pricing. These programs generate real benefit for the patients they reach, but they are frequently structured as time-limited pilots tied to specific drugs or countries rather than durable infrastructure, which means their impact tends to evaporate once the pilot period ends or the sponsoring company’s strategic priorities shift elsewhere. The patients and health systems that benefit most sustainably are those where pilot programs have been deliberately paired with domestic policy change, such as a temporary access program used explicitly to build the diagnostic and clinical infrastructure a country needs to eventually run its own reimbursement pathway.
What genuinely sustainable regional access would require, based on the clearest successes to date, is a shift from isolated interventions, whether government-run diagnostic pilots or company-run access programs, toward integrated systems that connect diagnosis, specialist referral, and treatment access as a single continuous pathway rather than three separate problems addressed by three separate actors. Taiwan’s model, which combines a national rare disease registry with an integrated referral and reimbursement pathway, offers the clearest regional template, though replicating it requires a level of sustained government investment and administrative coordination that many lower-income Asia-Pacific health systems currently lack the fiscal capacity to match without external support.
Closing the 250-million-patient gap will not happen through any single policy lever or corporate initiative; it requires coordinated investment across government genomic infrastructure, formal orphan drug regulatory pathways, and biopharmaceutical access programs explicitly designed to build durable domestic capacity rather than deliver one-off treatment access. The countries and companies that treat this as infrastructure-building, on a multi-year timeline, rather than as a series of discrete pilot programs measured by patients reached in a single year, are the ones most likely to actually move the region’s rare disease outcomes rather than simply publicize incremental progress against an enormous and largely unaddressed need.
