PCMHs emphasize continuous care, with a dedicated care team that includes primary care providers, specialists, and support staff. This model facilitates regular follow-ups, personalized care plans, and proactive management of diabetes-related complications. In countries like Sweden, healthcare providers are rewarded for achieving specific health outcomes for their diabetes patients, such as improved glycaemic control and reduced hospitalizations. This approach encourages providers to focus on preventive care and effective management strategies, ultimately leading to better patient outcomes and reduced healthcare costs.

In summary, innovations in diabetes service delivery are essential for addressing the growing burden of diabetes worldwide.
