For years, dengue in Hyderabad followed a familiar rhythm: cases climbed with the monsoon, peaked around its heaviest weeks, and tapered off once the rains stopped. That rhythm appears to be breaking down. Health authorities in Telangana are now warning that dengue is turning into a year-round public health challenge, and the reason has less to do with how much rain the city gets than with how that rain is arriving.
The alarm was triggered in part by a Union Health Ministry advisory flagging the possibility of an early dengue surge this year. But state disease surveillance officials say the underlying driver is a shift in the monsoon pattern itself — one that happens to suit the biology of disease-carrying mosquitoes almost perfectly.
Why broken rain is worse than heavy rain
Counterintuitively, a monsoon full of intense, continuous downpours is not the worst-case scenario for mosquito-borne disease. Heavy, sustained rain tends to flush mosquito eggs and larvae out of standing water before they can develop. What actually favours the insects is rainfall broken up by dry spells — exactly the pattern Telangana has been experiencing this year, according to a senior official with the state’s Integrated District Surveillance Programme (IDSP). Water collects during a shower, then sits undisturbed through the dry days that follow, giving mosquito eggs and larvae the uninterrupted time they need to mature.
That timing problem is compounded by where the water collects. Construction sites, building terraces and the flat roofs of government offices across Hyderabad routinely trap clean, stagnant water — precisely the environment Aedes aegypti, the mosquito species responsible for dengue transmission, prefers for breeding. Unlike species that thrive in dirty or open water bodies, Aedes aegypti favours small, clean, often overlooked containers: discarded tyres, coconut shells, flowerpots, construction debris. That makes it a difficult species to control through large-scale interventions like fogging, which are more effective against mosquitoes breeding in open, visible water.
The infections nobody sees
A second complication, officials say, is largely invisible: a substantial share of dengue infections produce no symptoms at all. Asymptomatic carriers don’t show up in hospital admission data or fever surveys, yet they remain part of the transmission chain, allowing the virus to keep circulating in a population even during weeks when reported case counts look reassuringly low. That mismatch between what surveillance systems can measure and what’s actually spreading in the community is one reason health officials describe current case counts as likely undercounts of the true burden.
An old problem, a newer set of tools
None of dengue’s basic biology has changed — but the conditions it depends on are shifting under the combined pressure of erratic rainfall and rapid urban growth, a pattern researchers have also documented in cities well beyond Hyderabad, where rising temperatures and unpredictable rain are turning what used to be a seasonal disease into something closer to a year-round threat.
Civic agencies still rely heavily on decades-old control measures — chemical fogging, oil-ball larvicides, pyrethrum spraying — but entomologists have raised a further concern: mosquito populations in the region are showing increasing resistance to some of the insecticides used in these campaigns, blunting their effectiveness over time.
For now, the most reliable defence remains a low-tech one. Health officials are urging residents to inspect and empty water-holding containers — flowerpots, coolers, buckets, discarded tyres — regularly, and especially during dry spells between rain showers, which is exactly when unnoticed breeding is most likely to take hold. With Telangana’s monsoon expected to stay erratic in the coming weeks, officials say that vigilance, more than any single intervention, will determine whether the city avoids a repeat of past years’ sharp case surges.
