The UDID (Unique ID for Persons with Disabilities) card serves as a single document for identification and verification of individuals with disabilities, facilitating their access to various benefits. The Department of Empowerment of Persons with Disabilities launched the UDID project to enhance transparency, efficiency, and the delivery of government benefits to people with disabilities. This initiative aims to streamline the process of verifying disability status and ensuring uniformity in accessing benefits.
Individuals with disabilities who wish to apply for a UDID card can do so by a comprehensive online application process. The process involves registering on the UDID web portal and providing personal details. Additionally, applicants are required to submit essential documents. Once the application and documents are submitted, they undergo verification by the CMO Office/Medical Authority.
Following verification, a specialist doctor assesses the disability and assigns a percentage. The Medical Board then reviews the case, and upon confirmation, the CMO Office prepares the Disability Certificate and generates the UDID. Subsequently, the UDID datasheet is sent for card printing, with the final UDID card being delivered by post to the provided address.
Applicants can track the status of their application by visiting the UDID web portal. This process aims to streamline and simplify the application for UDID cards, ensuring efficient delivery and access to benefits for persons with disabilities.
Procedure & Guidelines
Who Can Apply for a UDID Card?
All persons with disabilities can apply for it.
Step 1: Visit the UDID Web Portal and click on the register link to register.
Four types of details are required:
- Personal Details including Address
- Disability Details
- Employment Details
- Identity Details
Step 2: Use the PwD (Persons with Disabilities) login credentials to click on “Apply online for Disability Certificate” and enter the required details.
Step 3: After entering details, upload a color passport photo and other requisite documents such as identity proof, income proof, SC/ST/OBC proof, etc., as per requirements.
Step 4: Submit all the data to the CMO Office/Medical Authority for verification.
Step 5: The CMO Office/Medical Authority will verify all your data.
Step 6: After verification, the CMO Office/Medical Authority will assign the concerned specialist(s) for assessment.
Step 7: A specialist doctor assesses the disability of PwD and gives an opinion on the disability.
Step 8: The Medical Board reviews the case and assigns a disability percentage. The CMO Office prepares the Disability Certificate and generates UDID and Disability Certificate.
Step 9: The UDID datasheet goes for UDID Card printing, and the UDID card will be delivered by post at the address mentioned during online registration.
Documents Required with the Online Application
– Scanned copy of the recent colour photo
– Scanned image of signature (Optional)
– Scanned copy of Address Proof (Aadhar/Driving License/State Domicile, etc.)
– Scanned copy of Identity Proof (Aadhar Card/PAN Card/Driving License, etc.)
– Scanned copy of Disability Certificate (Only for those Persons with Disability who have been issued a disability Certificate by the competent Authority)
